Monthly Report Forms

These forms are used by probationers placed on administrative supervision. Offenders are required to mail, or drop off one form each month while under supervision.

Rules of Probation

This is the standard form for conditions of probation. This does not include any special conditions of probation that may be ordered.

Pre-Sentence Investigation Packet

This packet is used for offenders who have been ordered to submit to a pre-sentence investigation report.

Petition and Order for Travel outside of Indiana

This form needs to be filed with the court before any offender can leave the state of Indiana .

This is an outline of the financial impact on a person placed on probation. This is strictly the probation fee breakdown and does not include any other programming.
